• Certificate from respective Union Parishad/Municipal Chairman/Ward Commissioner with the mention of nationality, marital status and permanent address.
• Character certificate from class-I gazetted officer.
• 03 copies of attested recent passport size photographs.
• Character certificate from the head of the institute last attended.
• Certificate of permission from respective office/institute for the serving candidates (if applicable).
• Copies of original and attested equivalence certificate of educational qualification from respective equivalence committee (if applicable).
• The certificates of sports achievement at District / Devisional level should be brought along with (if any).
